* UAC applicants are required to obtain a Dun and Bradstreet number. A DUNS or D&B number tracks tupes of businesses in geographical locations. This number is free and is being used as an important advocacy tool for the arts. Please note this application will be accepted if you have applied for a Dun & Bradstreet number but have not yet received it. Please go to http://arts.utah.gov/funding/index.htmlfor step-by-step directions on applying and to get the Dun & Bradstreet website. Please note: the instructions and application for individuals and organizations are different – please choose the appropriate directions and application.
